President Trump said Friday that more than 20 million Medicare Part B enrollees will receive one-time $90 payments this month to offset premium costs, according to the New York Post.

Who qualifies for the $90 payment?

The White House says eligible Medicare Part B enrollees will get the money by direct deposit or paper check, the Post reported. Seniors whose premiums are already covered by Medicaid, or who pay an Income-Related Monthly Adjustment Amount because of higher income, do not qualify, according to the administration.

Where is the money coming from?

The payments are funded through the Medicare Improvement Fund, a pool created in 2008, the Post reported, citing the White House. Trump described the fund on Truth Social as a "pointless 'Slush Fund' that has only been used by Dumocrats in Congress to pay for the Waste, Fraud, and Abuse for their Special Interest friends, and drive up Healthcare costs," according to the Post.

The president framed the new payments as a redirection of that money toward beneficiaries rather than what he characterized as congressional misuse. "We are finally using this Fund, along with my Most Favored Nations Deals, to substantially LOWER costs for our Seniors," Trump wrote, per the Post.

What else has Trump said about the announcement?

Trump tied the payment to his broader record on entitlement programs in the same Truth Social post. "I pledged to protect and preserve Medicare and Social Security, which were on the verge of RUIN by Joe Biden, who foolishly and maliciously let tens of millions of Illegal Aliens into our Country, and we are doing just that for America's Seniors," he wrote, according to the Post. The Post did not report independent verification of the claim that Medicare and Social Security faced imminent collapse under the prior administration.

How does this compare to the Obamacare payments announced the same week?

The Medicare announcement follows a separate White House move disclosed this week offering $500 checks to roughly 950,000 Affordable Care Act enrollees, the Post reported. The administration has said those payments stem from what it calls overcharging of user fees to health insurers under the Biden administration, costs the White House says were passed on to consumers through higher premiums. The Post did not report a response from the Biden administration or outside verification of the overcharging claim.

What to watch

  • Whether the Centers for Medicare & Medicaid Services issues a formal rule or guidance detailing exact payment timing and eligibility verification.
  • Whether congressional Democrats, who Trump said used the Improvement Fund for "Waste, Fraud, and Abuse," respond to the characterization.
  • Whether the $500 ACA payments and the $90 Medicare payments draw separate legal or budgetary scrutiny given they rely on different funding mechanisms.
  • Whether income-based exclusions for IRMAA payers and Medicaid-covered seniors generate complaints from advocacy groups once checks go out.

The Post reported the announcement came late Friday and did not include a specific date by which seniors should expect to receive payment.
